POLICY |

The Day centre is Commonwealth funded for Day Therapy and
State funded for Day Care provides a wide range of therapy
services to frail older people living in the community and
funded residential aged care facilities.

Our aim is to assist older people to regain and or maintain
a level of independence which will allow them to remain in
the community as active, valued participants.

We focus on rehabilitation and we help access support from
other relevant services.

ELIGIBILITY |

Services are offered to people over 60 years and on a
pension living in the community of Port Lincoln and
surrounding districts.

REFERRAL |
-
Can be made by a professional person, carer, friend or by
yourself.
|

ASSESSMENT |

After referral the Coordinator will attend to an assessment
and discuss the needs of the clients.

The client is involved in the implementation of a care plan
and an agreed set of goals.

FEES |
-
A full list of fees are available from the Manager of the
Day Centre and are necessarily designed to be affordable for
all.

Clients who are unable to pay the fee will not be refused
access to services.

The revenue generated through fee charging is used to
maintain or expand the services.

RIGHTS &
RESPONSIBILITIES

All clients have the right to privacy relating to their
personal affairs including disclosure of their means/income to
the Day Centre for the purpose of assessing the client’s
capacity to pay.

Information pertaining to individual clients is stored in a
manner that maintains client confidentiality.

The Day Centre will inform clients of their rights and
responsibilities prior to the client accepting services, and
beginning the use of services.
