Matthew Flinders Support at Home Services enable choice, control and flexibility surrounding care for loved ones in their own home enabling them to feel confident, in control and ultimately, in their home for longer.
We help support and empower you and your loved ones to make your own choices and to design a plan for individualised support based on their care needs, goals and interests. From social outings, personal care, nursing care, transport and more, we do it all in a friendly and professional manner.
How do you access a Support at Home Package?
Eligible recipients should be 65 years or over, or 50 years and over for Aboriginals and Torres Straight Islander people.
In order to access a Support at Home Package, a person needs to be assessed and approved as eligible by an Single Service Assessment.
