Home Care Packages
Matthew Flinders Home Care Packages enable choice, control and flexibility surrounding all service provision. We operate in accordance with the Consumer Directed Care Principles and facilitate a regalement approach for all care.
We will support and empower you to make your own choices and to design a plan for individualised support based on your personal goals, interests and care needs. We support you to remain living independently in your own home for as long as possible, to continue to be actively involved with your community and to maximise quality of life by nurturing and maintaining your independence and wellness. Level 1, 2, 3 and 4 packages are provided
Our packages are designed for the individual by our experienced and qualified staff. From social outings to nursing services and transport, we do it all in a friendly and professional manner. Servicing both Private and Government Funded Home Care Packages.
How do you access a home care package?
Eligible recipients should be 65 years or over, or 50 years and over for Aboriginals and Torres Straight Islander people.
In order to access a Home Care Package, a person needs to be assessed and approved as eligible for Home Care by an Aged Care Assessment Team (ACAT). You can contact My Aged Care on 1822 200 422 or visit their website at www.myagedcare.gov.au for assessment and a referral to ACAT.
